Lab fair forms can be found at https://www.clarabartonhospital.com/lab-fair by clicking on the Lab Fair Form button or by visiting the front desk at Clara Barton Hospital. It is recommended that you fast after midnight and arrive with your lab fair form filled out and ready to go. Be sure to check the "send a copy of my results to my provider" box if you wish to have your provider receive your results. Lab results will be mailed out to you and your provider (if you choose) 10-14 days after the lab fair
